- Fill in the booking form and a representative of Tree Top Hospital will contact you
- Ensure that you bring your National ID/Work Permit/Passport to Tree Top Hospital for sample collection.

- Urgent/emergency report requests will be facilitated at a premium
- Onsite/Home visit sample collection can be facilitated as an additional service
