A
major surgical milestone was reached as our team performed a highly complex
oral and maxillofacial reconstruction, the first of its kind at our hospital.
As one of the longest procedures in our history, this achievement reflects our
advanced expertise in managing severe maxillofacial trauma with precision,
innovation, and seamless multidisciplinary collaboration.
The
patient presented with extensive facial injuries following a road traffic
accident. Advanced radiographic evaluation revealed multiple complex fractures
involving the orbit, maxilla, zygoma, and the base of the skull, as well as an
optic canal apex fracture causing critical nerve compression. These injuries
posed life-altering risks, including permanent vision loss, facial asymmetry,
and severe intracranial bleeding and neurosurgical complications.
Upon
admission, our ER team immediately activated trauma protocols. The patient was
stabilized in the ICU under neurosurgical care, while the Ophthalmology team conducted
a rapid assessment to address the progressive blurring of the patient’s vision.
Because time was of the essence, a multidisciplinary team was formed to
fast-track surgical management, supported by our expert Anesthesia and surgical
nursing teams.
The
Surgery was led by Dr. Mohammed Salman Basha (Consultant Oral &
Maxillofacial Surgeon) and assistant surgeon Dr. Aminath Reesha
(Dentist). The team performed an Open Reduction and Internal Fixation (ORIF) of
the fractured orbit, maxilla, and zygoma, alongside a decompression and
reconstruction of the orbital floor.
Utilizing
a state-of-the-art titanium Osseo-synthesis system, the team restored the
patient's anatomical integrity. In a sophisticated move to preserve facial
aesthetics, the surgeons strategically utilized existing lacerations as access
points, minimizing the need for additional incisions. The procedure lasted
approximately 7–8 hours, reflecting the intricate and demanding nature of the
reconstruction.
This
success highlights our growing capability to manage high-level trauma cases
locally; procedures that previously required treatment abroad. One week
postoperatively, the patient was discharged in stable condition with restored
facial symmetry.
This
milestone stands as a testament to our evolving standards of healthcare in the
Maldives and our steadfast commitment to delivering cutting-edge, life-saving
treatment for our community.
